Getting your records
Eaglesoft
Patterson Dental · installed in the office
Patterson’s installed system. Documents live in SmartDoc and images in Advanced Imaging. Everything else is a report, and any report preview can be saved to SmartDoc as a PDF and exported from there.
What to ask for
Send this to the office. It is written for someone who works in Eaglesoft every day, so there is nothing for you to translate.
- SmartDoc: export each of the patient’s documents with File | Export (Legacy SmartDoc on version 24.20 and later).
- Advanced Imaging: File | Export All for each exam, as image files, with the exam date noted for each folder.
- Perio: Print Preview each exam, send it to SmartDoc with the SmartDoc button, then export the PDF.
- Patient History report and Note History for all dates, saved to SmartDoc as PDFs and exported.
- Medical history: Print Form for each exam date.
- Lab reports: any the office scanned are SmartDoc documents. Eaglesoft’s Lab Tracking records cases sent to a dental laboratory.
Where it is in Eaglesoft
Each of these produces one part of the record. The office does not need to do all of them — only the ones you asked for above.
Documents (SmartDoc)
- Path
- With the patient selected: Activities | SmartDoc. On Eaglesoft 24.20 and later the new SmartDoc window opens first; click Launch Legacy SmartDoc in its upper-right corner, because export and email are only in Legacy SmartDoc. Select the document → File | Export → in Save As choose the location and name the document → Save. Repeat per document. To email instead: open the document → File | Email (needs a mail program such as Outlook).
- Produces
- The file as stored. "Eaglesoft reports saved to SmartDoc are saved as .PDF files." PDF and JPG are always allowed; DOC, XLS and HTML only if the office’s SmartDoc preferences allow them. The file name is whatever the office types in Save As.
- Choose
- File | Export, never a print or a screenshot. Ask the office to put the patient’s name and the document’s date in each file name.

Images and X-rays (Advanced Imaging)
- Path
- Activities | Imaging | View Images (or the Advanced Imaging icon; from the Scheduler, right-click the patient → Clinical | Advanced Imaging). Open the exam. One image: File | Export → location and File Name → Save as type "JPEG File Interchange (*.jpg)" → Save → Bits Per Pixel 8 for an X-ray, 24 for a color photo. Whole exam: File | Export All ("It will add a number after the name for each image in the exam"). Across all exams: File | Browse Images → Ctrl-click the images → File | Export All. The exam as one mounted page: File | Print All → a PDF printer.
- Produces
- Image files, one per image. Save as type also offers .PSD, .PCT, .TIF, .TGA, .BMP, .WMF and .PCX. DICOM is not in the list; Patterson sells a separate DICOM module. Patterson’s own instructions have the office type the patient’s name and X-ray date into the email subject, so do not expect them in the file.
- Choose
- Export All for the original images, as .TIF if the office can pick it, otherwise .JPG at 8 bits for X-rays. Print All to a PDF is a second copy of the mount, not a replacement. One folder per exam, named with the exam date.

Periodontal chart
- Path
- From the Clinical Activities menu choose Perio (or click the Perio icon). For an older exam use Edit Previous under the Perio tab. Click Print Preview. In the Print Preview window choose the SmartDoc button → enter the patient → change the report name to Perio Chart and the date → OK. Then export it from SmartDoc (File | Export). Repeat per exam. The Comparison tab’s Print button prints the Perio Comparison.
- Produces
- A PDF per exam in SmartDoc (reports saved to SmartDoc are PDFs). No XML or CSV export of perio values is offered from any menu.
- Choose
- One PDF per exam date, plus the Comparison printout.
Treatment history and clinical notes
- Path
- Completed services: Reports | Patient → Patient History → select the patient and a wide date range → Preview → SmartDoc button → PDF. Some reports also offer right-click | Save Data As → .csv; try it. Clinical notes (v20 and later): the Note Hist icon on the Clinical toolbar (or Activities | Note History) → set the Note Ledger filter to all dates and note types → Print → "print the report, Send via Email, or Export the report" (PDF). Also under Reports | Patient: Patient Notes Master and Patient Notes Audit Trail. The Clinical Exam’s History button prints "a detailed history of everything that has been entered in Eaglesoft".
- Produces
- Patient History as PDF: date, provider, service code, description, tooth, surface and fee. Notes as PDF from Note History’s Export, or by the SmartDoc button on any report preview. The patient’s name and the dates are printed in the report body.
- Choose
- The Patient History PDF for all dates, and the Note History export for all dates. The CSV is a bonus where the report offers it.
Medical history
- Path
- Click the Medical History icon in the main toolbar (or press F12 from SmartDoc, the Chart, Perio or Note History) → select the exam date in the lower left → Print Form ("prints form with patient’s name and DOB") or Print Answers. With File | Preferences | SmartDoc set to save Medical History printouts, the print lands in SmartDoc as a PDF; export it from there.
- Produces
- A printed form with the patient’s answers, one per exam date.
Lab cases
- Path
- The Lab Tracking icon in the Patient Bar opens the patient’s Lab Tracking window (or Activities | Lab Tracking). Tick Include Closed Cases and Include Returned Cases → select the case → Edit → Print (the Lab Case Detail report) → SmartDoc button → PDF → export from SmartDoc.
- Produces
- A Lab Case Detail report per case. Any lab paperwork the office scanned is a SmartDoc document.
A table as CSV (Database Navigator)
- Path
- Signed in as an administrator: "From the Practice management Screen, go to Utilities | Database Navigator." → "From Database Navigator, go to File | Open. Browse to the location of the SQL file - Select the SQL File | Open." → "The text will appear in the Command section of Database Navigator | Select Execute." The file lands wherever the script writes it; in Patterson’s own example, "The file will save in the C:\Eaglesoft folder as Prescriptions.csv".
- Produces
- A CSV with whatever columns the script selects. Patterson publishes scripts for particular jobs, such as the prescription export. No published script produces a whole single-patient record, the perio chart or the notes.
- Choose
- Only run a script Patterson supplied or reviewed. This is not a place for the front desk to write SQL.

What the office has to ask Patterson Dental for
These cannot be exported from the software itself. If you need them, say so early — the vendor is a second queue.
- Perio, notes or a complete single-patient record as data (XML or CSV). No menu offers it, and no published Database Navigator script produces it. Ask Patterson for a script.
- DICOM. It is not in the Export list; Patterson sells a separate DICOM module. Ask whether the office has it.
- A whole-patient SmartDoc export. Patterson documents one document at a time.
You may be able to get some of it yourself
Eaglesoft has no patient download of records; Patterson’s Patient Portal add-on lets you review your demographic information, message the office, and view and print forms you submitted.
True of any office
Whatever software they run, these are worth asking for.
- Documents and scans as PDFs: letters, referrals, treatment plans, consent forms, insurance explanations.
- Images and X-rays as the original files (JPEG, PNG, TIFF, DICOM), not photographs of a screen or a printout.
- The periodontal chart as data (XML or CSV) if the software offers it, and as a PDF if not.
- Treatment history: the list of procedures with dates and codes, exported as data if the software offers it.
- Lab reports as the PDF the laboratory sent.
