Getting your records
Dentrix
Henry Schein One · installed in the office
Henry Schein One’s installed dental system. There is no single "export this patient" button; each kind of record comes out of its own module, and images come out of whichever imaging program the office runs.
What to ask for
Send this to the office. It is written for someone who works in Dentrix every day, so there is nothing for you to translate.
- Document Center: export the patient’s documents (PDF).
- Images and X-rays: export them as image files from the imaging program (Dentrix Imaging Center, the older Dentrix Image, or the office’s own imaging software).
- Perio Chart: save each exam as XML, not only as a PDF.
- Patient Chart: print all progress notes, with procedure notes and clinical notes, to a PDF, and copy the progress notes to a spreadsheet.
- Ledger: copy the transactions to a spreadsheet, and print the Patient Ledger with ADA codes in the descriptions.
- Lab case prescriptions, referral letters, and anything scanned into the Document Center. Ask as well how they produce the registration details from the Family File; the vendor documents a Word letter merge for that, not a plain print.
Where it is in Dentrix
Each of these produces one part of the record. The office does not need to do all of them — only the ones you asked for above.
Documents (scans, letters, EOBs, forms)
- Path
- Document Center → select the document in the Document Tree → File → Export Document(s) → the Export Document As window → change the file name or extension if needed → Save. Several at once: select them all → File → Export Document(s) → the Export Documents window → choose the destination folder → OK.
- Produces
- PDFs. Most are scanned images with no text layer. The file name usually carries the date (10_16_2020); the PDF also holds a creation date, which is often weeks after the visit. The default file name is the document’s description; the Document Center’s reference date is not written into the file.
Images and X-rays (Dentrix Imaging Center)
- Path
- Patient Chart → select the patient → double-click the image in the Smart Image panel (or click the Launch Dentrix Imaging toolbar icon) → Images tab → select an image or a series → Share (or Share exam from the options menu) → in the "Exam or Image(s) to be shared" dialog set the options → Export.
- Produces
- JPG or PNG files; several images arrive in one compressed folder. A CAD/CAM scan downloads as a .ply file. The vendor’s guide does not say whether the patient’s name or the acquisition date are in the file name, so the office should note the date of each series.
- Choose
- Clear "Export as JPEG (Otherwise PNG)" to get PNG. Choose "Entire exam in separate image(s)" so each X-ray is its own file. Clear "Include image enhancements" for the unenhanced image.

Images and X-rays (older Dentrix Image)
- Path
- Dentrix Image → select the images → export.
- Produces
- JPEG files, occasionally BMP. No date is stored in the file and none is in the file name, so the office should note the date of each series or the patient will be asked for it.
- Choose
- Original exported images. A screenshot or a printed sheet loses the detail a clinician needs.

Images shown in the Smart Image panel
- Path
- Patient Chart → Imaging (Smart Image) panel → click the image → the Diagnostic Viewer opens → click the "Click to export this image" icon → save it as a file (the same icon offers copy to the Clipboard or send in an email).
- Produces
- One image file per export. The vendor page does not state the file format, and it does not say whether the image came from Dentrix Imaging or from a bridged program (DEXIS, Schick, Carestream, Planmeca). Those programs have their own export as well; ask which one the office uses.

Periodontal chart
- Path
- Perio Chart → File → Open Exam → select the exam → Open. File → Print Options → check Print Preview → OK. File → Print Perio Exam → Combined Graphic & Data Chart → Print. In the preview window, Save as → choose the file type. Repeat for each exam.
- Produces
- The save dialog offers Crystal Reports (.rpt), PDF, CSV, XLS, DOC, RTF and XML. That format list is what an office running Dentrix reported seeing in the preview window, not something the vendor’s help documents; the help covers the print and not the save. On the reports we have read, the XML keeps each field as data and the CSV is a page layout that loses most of it.
- Choose
- XML. A PDF is fine as a second copy for reading, but only the XML can be turned back into data.
Treatment history and clinical notes (Patient Chart printout)
- Path
- Patient Chart → select the patient → File → Print → Patient Chart. Under Print choose All Progress Notes. Click Select Details and keep procedure notes and clinical notes checked (clear Amount for a records transfer). Set the Patient Privacy boxes as the patient wishes. Click OK. The printer is set in the Patient Chart under Setup → Printer; pick a PDF printer or the Document Center Printer Driver there.
- Produces
- A printed chart: the graphic tooth chart and dated progress-note lines (procedure date, tooth, surface, code, provider, description, status, amount). The header can hide the SSN, chart number and birth date. Printed to the Document Center Printer Driver, it lands in Unfiled Documents as a PDF; from there, export it like any document.
- Choose
- All Progress Notes, with procedure notes and clinical notes included. Save Options (for example "Patient Leaving") keeps the settings for the next request.
Procedure history as data (the one Dentrix export myDxHub reads back into a chart)
- Path
- Open the procedure history report the office normally prints (Office Manager → Reports, or the Patient Chart’s procedure list) → Print Preview → Save as → XML.
- Produces
- A Crystal Reports XML file carrying one row per procedure: date, tooth, surface, ADA code, provider, description, status and fee, with clinical notes attached to the rows that have them. myDxHub reads this file and builds the patient’s treatment history from it; every other Dentrix export is stored as a document.
- Choose
- XML. Ask for this one first: it is the only Dentrix export that becomes structured data rather than a picture of a page.

Progress notes as data (to a spreadsheet)
- Path
- Patient Chart → View → Progress Notes → choose the view (View All is the default) → Options → Copy Progress Notes to Clipboard → paste into a spreadsheet → save as CSV.
- Produces
- The displayed progress notes as text: "Paste the progress notes into any program compatible with the Clipboard." The vendor does not say which columns are copied, so check the paste against the screen and put the patient’s name in the file name.
- Choose
- Keep this alongside the printed chart; the paste is the copy a computer can read.
Clinical notes
- Path
- Patient Chart → View → Panels → Clinical Notes → Print in the Clinical Notes toolbar → Date Range: All Dates; Provider: <ALL>; Patient: Current Patient → OK. For the text itself, click Copy All Clinical Notes in the same toolbar and paste it.
- Produces
- A printed list of dated, signed clinical notes, or the note text on the Clipboard. The vendor does not mention PDF; a PDF printer is the office’s choice.
Account and ledger history (procedures, payments, adjustments)
- Path
- Ledger → select the patient → Options → Ledger + History, which shows "all transactions for the selected patient, including transactions in history". For the data: File → Copy to Clipboard, which copies "all displayed transactions" → paste into a spreadsheet → save as CSV. For the report: Print → Patient Ledger (or Family Ledger) → in the Patient Ledger Report dialog set the First Transaction Date back far enough to cover every visit → Print, or Send to Batch. The same two reports are in Office Manager under Reports → Ledger → Patient’s Ledger Report and Family Ledger Report. To put procedure codes in the descriptions: Office Manager → Maintenance → Practice Setup → Preferences → Print Options tab → Use ADA Codes in Descriptions.
- Produces
- The pasted transactions, one dated row each, as a spreadsheet. The Patient Ledger report prints on paper, or to a PDF printer. Print → Walkout gives the per-visit procedure statement instead.
- Choose
- Copy to Clipboard for the data, and the Patient Ledger report with ADA codes turned on as the formal copy. The copied rows are only what is on screen, so put the patient’s name in the file name.
Treatment plan (Treatment Planner)
- Path
- Select the patient in the Patient Chart or Ledger → open the Treatment Planner → File → Print → Print Treatment Case → pick a profile under Select Report Options → under Print check Treatment Case Note, Graphic Chart and Signed Consent Forms → under Case Procedure Options check Include Procedure Notes → Select Columns → set the Patient Privacy boxes → Print Preview → OK.
- Produces
- A printed treatment case with the case note, one line per procedure (date, tooth, surface, code, provider, description, fee, patient portion, insurance portions) and any signed consent forms. The vendor says nothing about PDF; it depends on the printer the office picks.
Health history, questionnaires and patient notes
- Path
- Health History: in any module except the Office Journal and eDex, File → Switch To → Health History → select the patient → Print on the toolbar. Submitted questionnaires are listed in Health History; double-click one to open the Questionnaires module and print it there. Patient notes: Family File → select the patient → File → Print Patient Notes → tick the note types (Patient Alert Notes, Perio Exam Notes, Health History, Lab Case Note and the rest) → Print or Send to batch.
- Produces
- Printed reports: conditions, allergies and medications; each questionnaire as the patient filled it in; and the patient notes by type. Nothing structured.
Referral letters and other correspondence
- Path
- Letters the office filed are Document Center documents; export them with File → Export Document(s). To produce one again: select the patient → the Quick Letters toolbar button → "Referred to Dr./Specialist", or one of the referral thank-you templates → Build/View, which opens Word so the letter can be read first, or Print, which merges and prints it. "A message appears asking if you want to save the changes. Click No." An entry is added to the Office Journal automatically. For a referral summary rather than a letter: Patient Chart → File → Print → Referral Slip, which carries the patient, the provider, the referral information, treatment-planned and referred procedures for a date range, and notes.
- Produces
- A Word document, or a PDF if the office prints to a PDF printer. The only filing route the vendor documents is printing to the Document Center Printer Driver, which lands the letter in Unfiled Documents as a PDF.
- Choose
- The copies already filed in the Document Center. Regenerate from Quick Letters only when nothing was filed.
Lab cases
- Path
- Lab Case Manager → select the patient and the lab case → the Print Prescriptions button on the vertical toolbar → the Print Prescription(s) dialog → Print, or Send to batch. The case dialog itself also offers Print Case and Send Online. Lab Case Note is one of the note types on the Family File patient note report.
- Produces
- A lab case prescription: create date, provider, category, case note, enclosures, lab, shipping method and sent date, tracking number, shipping cost, case number, expected receipt date, and the procedures with their shade guide and shades. Dentrix holds no lab result. Paperwork the laboratory sent back is a Document Center document if the office scanned it.
Any Dentrix report as a PDF file
- Path
- Send the report to the Batch Processor. In Office Manager, select it → File → Print Preview → File → Print Report → choose the Document Center Printer Driver (or any PDF printer installed in Windows). Then Document Center → Unfiled Documents → Document Information → assign the patient, type and description → attach it → File → Export Document(s).
- Produces
- A PDF. Printing to the Document Center Printer Driver "will import the document into the Unfiled Documents window as a PDF".
- Choose
- A PDF printer is the direct route. The Document Center detour also files a copy in the patient’s record.
What the office has to ask Henry Schein One for
These cannot be exported from the software itself. If you need them, say so early — the vendor is a second queue.
- A whole-record export in one step. Dentrix has no "export patient" command; a full copy of the database comes through Henry Schein One or one of its authorized integration vendors.
- Original radiograph files (DICOM, or the sensor’s own format). Dentrix Imaging Center exports JPG or PNG only.
- Acquisition dates for images exported from the older Dentrix Image. The files carry none. Henry Schein One’s conversion of Dentrix Image data into Dentrix Imaging Center keeps "tooth numbers/anatomic region, date of acquisition".
You may be able to get some of it yourself
Dentrix itself has no patient download. Some offices add a separate portal product.
True of any office
Whatever software they run, these are worth asking for.
- Documents and scans as PDFs: letters, referrals, treatment plans, consent forms, insurance explanations.
- Images and X-rays as the original files (JPEG, PNG, TIFF, DICOM), not photographs of a screen or a printout.
- The periodontal chart as data (XML or CSV) if the software offers it, and as a PDF if not.
- Treatment history: the list of procedures with dates and codes, exported as data if the software offers it.
- Lab reports as the PDF the laboratory sent.
