Getting your records
Denticon
Planet DDS · cloud
Planet DDS’s cloud system. Documents are attachments on patient notes, images live in a separate imaging program (usually Apteryx XVWeb), and the rest comes out as PDF reports.
What to ask for
Send this to the office. It is written for someone who works in Denticon every day, so there is nothing for you to translate.
- Patient → Notes, filtered to all notes: save each attachment (scans, uploads, and saved consents and treatment plans).
- Imaging (Apteryx XVWeb, or the office’s own imaging program): export the images as DICOM in a ZIP; JPEG or PNG only if DICOM is not possible.
- Perio Chart: Print/Preview each exam date and save the PDF.
- Clinical History report with Progress Note, Pre-Existing Conditions and Prescriptions checked, saved as PDF; the Patient Ledger for the dated procedure list.
- Lab reports: Denticon has no lab results module; any the office kept are scanned notes. Ask them to include those.
Where it is in Denticon
Each of these produces one part of the record. The office does not need to do all of them — only the ones you asked for above.
Documents (scans, uploads, saved consents and treatment plans)
- Path
- Open the patient record → click Patient → Notes (or the Notes icon on the toolbar) → set the filter to show all notes, so saved PDFs such as signed consents and treatment plans appear (the Dashboard paper clip shows only uploaded and scanned attachments) → click the Attachment (paper clip) icon on the note to view it → save the opened file with the viewer’s own save. Repeat per document. A Denticon letter or treatment plan that was never saved: Print Reports → Letters or Treatment Plan → Print/Preview → Save PDF file. A statement: the Print menu → Statement → the statement date → Print/Preview → save the PDF.
- Produces
- The stored file: .pdf, .jpg, .jpeg, .png or .gif (the only upload types; 10 MB each). Office scans are saved as PDF. No bulk export: "If your practice needs a copy of patient notes (PDF files, JPEG files, etc.), please contact Denticon Support."
- Choose
- Save from the Notes screen, not the Dashboard, so saved consents and plans are not missed. Put the note’s date in the file name; the file carries none.
Planet DDS support: We attached a scanned document to a patient record. How do we view it?
Images and X-rays (Apteryx XVWeb)
- Path
- In Denticon, open the patient record → click the X-ray bridge (tooth) icon to launch the imaging program with the patient loaded. In XVWeb (the user needs the Export privilege: username → About and Help): Select → click the study thumbnails or images (an orange box marks the selection) → Export → Download → archive .ZIP (or .ISO) → file type JPEG, PNG or DICOM → optional zip password → Export. A large series appears under the notifications tab. Or Export → Email, with a link that expires in 1 to 7 days and a zip password. CT images are emailed one at a time.
- Produces
- A ZIP with one file per image as DICOM, JPEG or PNG. "DICOM files contain PHI within their metadata", so DICOM carries the patient and the capture date; JPEG and PNG do not. Export → Screenshot makes one composite image of the series with the patient name and tooth numbers shown.
- Choose
- DICOM in a ZIP. JPEG or PNG only if the recipient cannot read DICOM. Not Screenshot. If the office bridges other imaging software (Dexis, Schick, Sidexis, Carestream), the export happens in that program.
Periodontal chart
- Path
- Open the patient record → click Charting → Perio Chart → select the exam in the Date of Service dropdown → click the Printer icon → Perio Chart → in the popup select the recorded date → Print/Preview → the report opens in the Adobe screen → print, or File → Save (if the buttons are hidden, right-click the gray bar → Show/Hide Toolbar → File). Repeat per exam date.
- Produces
- A PDF report of one exam per run. The header fields are not stated. No per-patient CSV or XML export; perio values as data exist only in the whole-practice Denticon Download (Utilities → Denticon Download, sheet "Chart Perio Activity").
- Choose
- One PDF per exam date. Do not use Print Screen or the e-claim Add Perio Chart, which attach a picture of the entry screen.
Treatment history and progress notes
- Path
- Open the patient record → Print Reports icon → Clinical History → check Progress Note (and Pre-Existing Conditions, Prescriptions, and Print XRays if wanted) → set the dates → Print/Preview → Adobe screen → print, or File → Save. Notes older than about 10 years: Reports → Lists → Patient List → Patient’s Notes report → last name → Search → double-click the patient → dates → check Chart Notes → Print/Preview. Dated procedures with codes and fees: Print Reports → Patient Ledger → Print/Preview. Proposed treatment: Print Reports → Treatment Plan. Medical history and demographics: Print Reports → Medical History.
- Produces
- PDFs generated to the Adobe Reader screen: Clinical History, Patient’s Notes, Patient Ledger, Treatment Plan, Medical History. The graphic tooth chart cannot be printed; the vendor’s only route is the keyboard Print Screen.
- Choose
- Clinical History with Progress Note, Pre-Existing Conditions and Prescriptions checked, plus the Patient Ledger for the dated procedure list.
Planet DDS support: How can I print my patient’s progress notes
What the office has to ask Planet DDS for
These cannot be exported from the software itself. If you need them, say so early — the vendor is a second queue.
- A complete copy of the patient’s note attachments: Denticon Support, by ticket.
- A full image export from Apteryx (an Apteryx Image Data Request): a Planet DDS support ticket; delivered by SFTP as password-protected ZIPs of DICOM.
- Images held in a bridged imaging program (Dexis, Schick, Sidexis, Carestream, Planmeca) are exported in that program, not in Denticon or XVWeb.
- Denticon’s own email cannot carry attachments. Consents and treatment plans can be emailed through Message Hub; other files go from the office’s own email or on media.
You may be able to get some of it yourself
Denticon’s patient portal is for balances, statements, payments, appointments, medical history updates and consent forms; the vendor documents no patient download of clinical records or images.
True of any office
Whatever software they run, these are worth asking for.
- Documents and scans as PDFs: letters, referrals, treatment plans, consent forms, insurance explanations.
- Images and X-rays as the original files (JPEG, PNG, TIFF, DICOM), not photographs of a screen or a printout.
- The periodontal chart as data (XML or CSV) if the software offers it, and as a PDF if not.
- Treatment history: the list of procedures with dates and codes, exported as data if the software offers it.
- Lab reports as the PDF the laboratory sent.
